Hauling Sacks

Begin by hauling sacks to a shed. This is a mandatory task, so grab a sack from the wagon and take it to the shed near the fenced area. Drop the sack inside and repeat until the objective is complete.
Help the Blacksmith

Next, assist the blacksmith who, although reluctant, instructs you to craft a horseshoe. Enter the forge area through the archway opposite the blacksmith. Gather the necessary materials and create the horseshoe. Once approved by the blacksmith, follow the path you used to haul sacks to the stables and deliver the horseshoe.
Help Fanka the Cook

After delivering the horseshoe, focus on helping Hans. Return to the forge to retrieve a lockpick from the red pot on the shelves. Then, converse with Fanka, the cook standing nearby. By offering your help, you gain access to the noble's kitchen. Accompany Fanka to the kitchen; while not required, lockpicking the door for her boosts your reputation and grants you future access to food.
Speak with the Chamberlain

Upon entering the kitchen, you'll notice the chamberlain in distress. Engage with him and attempt to persuade him to allow you to brew a Digestive Potion. Success grants you access to the surgeon's workshop; otherwise, you'll need to sneak in.
Make Fever Tonicum

If you failed to convince the chamberlain, you'll need to use food from the kitchen to distract the guard in the non-noble kitchen across from the forge. This allows you to ascend the stairs to the surgeon's workshop. If granted access, head straight to the workshop. Inside, craft the Fever Tonicum and, if applicable, the Digestive Potion using the ingredients from the chest.
Cure Thomas

If aiding the chamberlain, return to him with the Digestive Potion to secure permission to help Thomas. If sneaking, proceed to the noble's kitchen and up the stairs to avoid additional guards. Stay crouched and use a pebble to distract the guard at the bottom of the staircase leading to Thomas' room. Sneak past him and reach Thomas, who is being cared for by his sister, Adele. Hand over the Fever Tonicum to Adele, triggering the quest's concluding cutscene.
